Doctor accused of leaving forceps inside patient during heart surgery
According to reports, 22-year-old Saqlain, a resident of Toba Tek Singh, was admitted to a cardiology institute for treatment of a hole in his heart. The surgery was reportedly performed on April 4 by cardiologist Dr Zaigham Rasool.
It is alleged that during the complex procedure, a pair of surgical forceps was mistakenly left inside the patient’s chest due to negligence. The issue remained unnoticed until the patient began experiencing severe chest pain days after the surgery.
Following his complaints, doctors conducted an X-ray which revealed the presence of the surgical instrument inside his chest. After the discovery, the medical team carried out a second operation on April 23 to safely remove the forceps.
Hospital administration confirmed that the matter came to light after the patient developed complications, leading to immediate action and an internal inquiry. Medical Superintendent Dr Nadeem stated that the investigation found a lapse in standard operating procedures, particularly in the counting of surgical instruments after the operation.

According to the inquiry findings, staff nurses failed to properly count and verify the instruments used during surgery, which resulted in the serious oversight. Following the investigation, both nurses involved in the procedure were held responsible and have been sent to the secretary’s disposal for further action.
The incident has raised concerns about patient safety and hospital protocols, with calls for stricter monitoring and accountability in surgical procedures to prevent such cases in the future.
